form for each sport. Entry forms may be photocopied, but
remember to copy both the front and back of the form.

the question(s) is applicable to your sport.

list partner(s) in the right-hand column. Each partner and
relay team member must be a registered athlete and must submit
their own, completed individual entry form.

or money order payable to the State Games of America or
by credit card (Visa, MasterCard or American Express).
7. Return the completed entry form(s), with payment, to the SGA
office via mail or fax. Online entries will only be accepted
with a credit card payment. SGA will only accept faxed entries
with credit card payment. Make sure you have read and signed the
waiver on the back of the entry form. A completed
entry form must include the waiver signature of the participant.
Parent/guardian signatures must also be provided if the
participant is under the age of 18. Team rosters/waivers must be
signed by each member of the team or a parent/guardian
if the participant is under the age of 18. All incomplete
entries will be returned for completion. Incomplete entries will
be
processed only after they are completed and returned. The State
Games of America will retain entry fees for forms that
remain incomplete.

a team sport. Each team member is required to sign the
roster/waiver on page 31 of the entry booklet, which MUST be
