State Games of the West Staff and/or Sport Commissioners reserve the right to combine and/or eliminate divisions/age classifications if an insufficient number of entries are received.

After the registration deadline, a waiting list will be established for Team Sports ONLY. If a team drops out, those on the waiting list will be called on a first-received, first-filled basis.

Registration Instructions
Each sport varies as to site, fees, age requirements, registration deadlines, and National Governing Body membership
requirements for competition. Refer to your specific sport profile for this information.

1. Turn to your sport profile and carefully read the entire sport description noting all events and divisions that are offered for
that sport. Determine the event(s)/division(s) for which you are qualified.

2. Remove the entry form from the center of this registration handbook. The entry form has two sections: one applies to individual sports and the other applies to team sports. If entering more than one sport, you must fill out a separate entry form for each sport. Entry forms may be photocopied, but remember to copy both the front and back of the form.

3. In the Event Information section, fill out the sport code, gender code, division code and event code for each event you or
your team are entering and write in the event description (i.e., 200 M hurdles). Please note: not all sports require a division
and event code.

4. Complete the “Sport Specific” section of the entry form if the question(s) is applicable to your sport.

5. For doubles events, mixed doubles events and relay events, list partner(s) in the right-hand column. Each partner and
relay team member must be a registered athlete and must submit their own, completed individual entry form.

6. Carefully calculate entry fees. Payment can be made by check or money order payable to the State Games of the West or
by credit card (Visa, MasterCard or American Express).

7. Return the completed entry form(s), with payment, to the SGW office via mail or fax. SGW will only accept faxed entries
with credit card payment. Make sure you have read and signed the waiver on the back of the entry form. A completed
entry form must include the waiver signature of the participant. Parent/guardian signatures must also be provided if the
participant is under the age of 18. Team rosters/waivers must be signed by each member of the team or a parent/guardian
if the participant is under the age of 18. All incomplete entries will be returned for completion. Incomplete entries will be
processed only after they are completed and returned. The State Games of the West will retain entry fees for forms that
remain incomplete.

8. Only one registration form is necessary when registering for a team sport. Each team member is required to sign the
roster/waiver on page 31 of the entry booklet, which MUST be sent in with the entry form.

9. Registrations must be postmarked by the entry deadline date specified on each sport page unless otherwise specified.

Most rules have been established by the National Governing Bodies; however, some competition rules or formats may have been altered to make them appropriate for this tournament.
